Overview
La hemostasia comenzó a tomar protagonismo en el síndrome de disfunción multiorgánica, generalmente asociado a CID y, más recientemente, ante patologías que afectan al paciente crítico, como la sepsis o el síndrome de respuesta inflamatoria sistémica.
EEl manejo de la hemostasia constituye, junto la estimulación de la eritropoyesis y la optimización de la tolerancia fisiológica de la anemia, un pilar fundamental en el concepto de Patient Blood Management (PBM), recientemente reconocido por la OMS. El enfoque multidisciplinar y multimodal supone un cambio de paradigma al centrarse en la identificación y provisión de los cuidados necesarios para la continuidad asistencial del paciente. En el PBM, la comunicación y coordinación entre las diferentes disciplinas mejora el resultado clínico y reduce los costes de tratamiento.
Esta monografía ha sido elaborada por un panel multidisciplinario de expertos, incluyendo intensivistas, anestesistas y hematólogos con amplia experiencia. Contempla variados escenarios clínicos del paciente crítico, múltiples etiologías (trauma, quemadura, sepsis, cáncer, disfunción multiorgánica) y diversas características de pacientes (adultos, mujeres y niños); con la finalidad de enfatizar que el modelo coagulopático común del paciente crítico presenta características diferentes en función de la etiología y, por tanto, requiere un abordaje específico, basado en la fisiopatología del cuadro.
